FIELD TRIP REQUEST FORM ~ New Guidelines~ As of January 18, 2008 Your field trip must have at least 25 students (ages 3 & up) in order to take advantage of these rates.

For every ten (10) students (ages 3 and up), one employee teacher/aide will be given one complimentary admission and one complimentary train ride (if the group purchases train rides.)
Each bus/van driver may also receive one (1) complimentary admission ticket
Our reservations personnel will email you a reservation confirmation and payment information.
To
take advantage of the prepaid rates, complete this form, make a copy for your
records, and mail a check payable to:

Prepaid rates must be paid to The ZOO one week in advance of your trip, or non-prepaid rates will apply.
